4 easy steps to a $200 Reward

Ready to finish out 2025 with a win?  To maintain your active status and continue working with your clients in 2026, all team members are required to complete their Annual Requirements by December 31st, 2025. 


We know this is not anyone's favorite part of the job. It takes time and focus. That's why we're recognizing your effort in prioritizing these professional standards by offering a Performance Incentive of up to $200 for getting them done early and adhering to PHCS policies.

2025.Annual Requirements

Quality Care Renewal

Here's the step by step guide to quickly complete your requirements for the year, and get that $200 incentive!


They're pretty much the same as last year and are designed to be as easy to use and meet as possible. As always, call for any questions - we're here to help!

Information Update

  • Complete the form to verify your contact information
  • Upload any documents that are expired to renew them

Policies Renewal

  • Review PHCS Policies and Standards
  • Read Annual Required Documents packet
  • Sign and submit the 2025 packet online

Annual Training
  • Review which courses are required for your position
  • Complete all relevant training modules
  • Pass all test with at least 70%
Qualifying for the Incentive
To qualify for the incentive, you must complete the requirements above by the deadline AND meet all PHCS Standards and Expectations for all of December 2025. (These are always required, should be pretty easy!)

  • Complete and pass all required trainings for your role by the deadline schedule.
  • Comply with all policies for reporting requirements, missed visits, UI/MUI, etc.
  • Accurately clock in and out for all visits requiring EVV according to your assigned schedule and matching your work report(s).
  • Report immediately to PHCS any complication that does not allow you to meet these expectations.
  • Submit all documentation and work reports on time
    • Remember: Digital forms and work reports are due at the end of the visit, on the same day.



Incentive Breakdown

Don't just renew your requirements, get a head start into 2026!

IMPORTANT: The annual requirements are MANDATORY to continue working next year. Finish early, and gain the incentive. If you don't finish in time, it's still required and you will not be eligible for the incentive.
