tax & payroll forms
To update your Tax and/or Payroll information, please go to your ADP app and login. There you are now able to update your bank account, change tax forms, see your paystubs, W2s , and more!
If you do not see these options within you ADP app, please call the office during business hours so we can assist.
For issues logging in to or accessing ADP, please speak directly to ADP. The ADP account is yours, through their company. We are not able to reset passwords, or access your personal account to make any changes.
Information update & Corrections
Links to the documents to formally make change and requests to your Team Member Profile, Hours, or status.
Team Member Documentation Update
Send updated versions of required documentation directly to PHCS:
- ID Card
- Auto Insurance
- CPR/First Aid
- License / certifications
- Other Team Member records
Schedule update
To change or update your preferred schedule or working hours.
Required for:
- All team members wanting to update availability for additional clients
EVV Corrections
To make or notify of changes required for missed or incorrect EVV Times
Required for:
- All team members using EVV when errors are noted
