Required for all Team Members.

IRS FOrm W-4 

Federal filing of withholding status. Required for: 

  • Name change
  • Change in dependents
  • Change in federal withholding amount
  • Change in filing / marital status

ADP Direct Deposit

Change where your paycheck is deposited. Required for: 

  • New Hires
  • Change in banking account

Note: Requires a least 2 weeks from date received to take effect.
