- Tax and Payroll Forms: Optional, unless there are changes or renewal requirements are not met.

Complete and submit an updated version of the the following forms only if there has been a qualifying change. See each item for specific details.
IRS FOrm W-4
Federal filing of withholding status. Required for:
- Name change
- Change in dependents
- Change in federal withholding amount
- Change in filing / marital status
Ohio it-4
State filing of withholding status. Required for:
- Name change
- Change in dependents
- Change in federal withholding amount
- Address change
ADP Direct Deposit
Change where your paycheck is deposited. Required for:
- New Hires
- Change in banking account
Note: Requires a least 2 weeks from date received to take effect.
