Here’s how to proceed:

• Review the Documents: Please take a moment to carefully scroll through all pages. Your understanding of the material is essential.
• Sign the Acknowledgment: After reviewing, click on "Start Signing" to provide your signatures where required.
• Submit Your Information: Once you’ve signed, simply click "Submit" or the pen symbol to send your information directly to PHCS.
• Continue to Part 2: After completing this step, click Continue to Part 2 to proceed
