 Here’s how to get started: (click on each underlined item for more)

  1. Annual Disclosure Documents:
    • Review and sign Abuser Registry, Ethics, Attestation, and TB forms.
  2. Team Member Update and Tax Form Review:
    • Submit short form to ensure your personal information is current.
    • Only complete tax or payroll docs if something has changed
  3. Verify Compliance:
    • Ensure you are compliant with access to and use of EVV, Digital Work Reports, and Reporting Policies. 
    • Sign the document for acknowledgement and understanding. 
  4. Annual In-Services
    • Complete all 2024 in-services as required by your position at PHCS.
      • Nurses: Take all mandatory course and the DODD training (if applicable)
      • Aides: Take all mandatory courses, the HHA Continuing Education, and DODD (if applicable)

Note: This is only for Team Members hired before Jan 1, 2024. If you were hired this year, there's no need to complete the annual renewal - you're already done!

Annual Documents
Review and sign the Annual Team Member documents. This includes the FCSO Ethics, Abuser Registry Annual Notice, the Attestation and Agreement to Notify and TB Screening. All are signed as one document, completed through JotForm.

Policies and Compliance
Review each of the 5 topics to make sure you are in compliance with each. Then, sign the embedded JotForm document to complete.  If you have questions or are not able to access something or comply, please call the office ASAP.

Annual In-Services
View and complete all applicable in-services for your role at PHCS. All required courses must be passed at 70% or better to qualify.

To Complete: 

  • Aides take courses marked as "All", "Aide", and "DODD" if applicable,
  • Nurses take courses marked as "All" and "DODD" if applicable.
